安寧療護多專科團隊會議的功能為精進臨床照護品質、培育安寧種籽專業人才、發現研究問題、促進團隊溝通、疏導成員情緒與個人成長。本文旨在以實際案例呈現安寧療護多專科團隊會議之運作模式，以成大醫院為例說明會議前中後過程，包括會議前選擇典範案例，準備簡報呈現病人全貌，擬定討論議題，邀請與會人員-病人原照護團隊及相關專家；會議中由安寧團隊召集人擔任可勝任的主持人，促進成員發言，引發討論以及總結會議結論，藉此與原照護團隊建立雙向溝通管道與合作關係，為病人及家屬共擬適切照護計畫，凝聚未來之照顧共識，進而提升臨床照護品質，以及培育安寧療護種籽專業人才，讓安寧的理念與實務擴散到各科。會議結束後，彙整會議記錄，做為未來的教學素材，落實以病人為師。期望本文能拋磚引玉，引發各界安寧先進共同思索精進安寧療護多專科團隊會議的實際運作模式與功能，持續提升會議效能，達到彼此交流與學習之目的。

The purpose of multidisciplinary team conference in palliative care is to advance quality of clinical care, educate health care professionals, identify research bottleneck, promote inter-team communication, and to encourage team member to express feelings. The aim of this article is to demonstrate how a multidisciplinary team conference in palliative care is conducted at National Cheng Kung University Hospital. The conference preparation starts from the model case selection, PowerPoint case presentation of patient's comprehensive history, discussion topics formulation, and invitation list of original care team and disease specialists, etc. In order to build up mutual communication and cooperation, the moderator is often the convener of palliative care team who can best guide the discussion. The conference records which are summarized by palliative care case managers could be used as teaching materials in the future. It is important to continue to strive for constant effectiveness of multidisciplinary team conference for palliative care team.
