出院是病人入院診療中重要的一環,完善的出院準備必須整合院內外相關資源,讓病人療程更為順利。而衛生福利部中央健康保險署為了讓病人在出院返家後,獲得連續性的照顧,同時也減輕家屬的負擔,推動出院病人之出院準備及追蹤管理計劃,鼓勵醫院在病人出院前做好出院準備及出院後追蹤諮詢。本院配合該計劃重整院內相關資源,成立出院準備及追蹤服務委員會及工作小組,目前可以承擔全院20%的出院病人的相關業務,並能百分之百完成收案服務。從業務量分析、病人滿意度及再入急診率統計,顯示服務成效相當良好。執行後的經驗讓我們得以進一步檢討出院準備及追蹤服務目前的作業系統以及工作流程;發現人員的教育、病人及照顧者的參與程度、評估工具等都是影響出院準備及追蹤服務品質的主要因素。藉此機會深入釐清問題,作為我們將來改善的依據與方向。 |
英文摘要 |
Discharge planning is extremely important in any hospitalization. It plays a critical role if a good quality transition from hospital to home is the purpose. This can only be accomplished by a successful collaboration between clinical and community-based service. To achieve the goal, a project initiated by the Taiwan National Health Insurance Administration has been launched to make sure that all issues related to patient's follow-up care are addressed before discharge. The healthcare providers in Koo Foundation Sun Yat-Sen Cancer Center, based on the project, were able to complete a comprehensive assessment regarding patient's discharge and provide a continuous care to meet their needs at home. According to our findings, not only patients were satisfied with our service, the rate of ER visiting 3 days after discharge was also decreased. The experience has enabled us to reconsider the appropriateness of the operating systems and process, to identify the contributing factors regarding the quality of service which includes education preparation of the staffs, involvement of patients and their family in the process, and the assessment tools used. All of these provide a great opportunity for us to improve the quality of service we offer to our patients and their family in the future. |
