本文旨在探討照顧一位子宮體癌症末期病人產生預期性哀傷的護理經驗，護理期間自2014年7月25日至2014年8月5日，筆者藉由照護過程中之會談、傾聽、觀察等收集主觀、客觀資料，並以生理、心理、社會、靈性之四大項進行整體性評估，發現個案的健康問題為：疼痛、低效性呼吸型態、預期性哀傷。筆者透過與個案及家屬之照護過程，除調整止痛藥物使用外，亦給予個別護理指導個案及家屬運用轉移注意力、芳香療法、按摩、氧氣療法、舒適臥位等技巧緩解個案疼痛及呼吸困難，並運用安寧療護之全人、全家、全隊、全程之四全照護，以陪伴、誘導性談話等建立治療性關係，提供個別性的護理措施給予正確訊息及建議，並引導生命回顧緩解個案及家屬預期性哀傷，以提升個案照護品質，最終完成後事準備以安然地面對死亡將至，藉此個案之安寧照護經驗分享，提供日後面對此類病人照護之參考。

This article describes the experience of patient care for anticipatory grief from end-stage uterine carcinosarcoma. Subjective and objective data were collected through interviews, listening and observations of physiological, psychological, social, and spiritual aspects during the period of July 25th to August 5th, 2014. Three health problems were identified: pain, ineffective breathing pattern, and anticipatory grief. Through the nursing process, we adjusted the analgesic drugs, provided entertainments as distractions, applied aromatherapy, massage, and oxygen therapy, and showed patients the comfortable positions to relieve pain and dyspnea. We also used the holistic hospice care with the approach of whole person, whole family, whole team, and whole disease to provide companionship and conversation guidance to establish therapeutic relationships. In addition, we provided individualized suggestions to give the appropriate information and suggestions to ameliorate anticipatory grief symptoms and improve quality of care for the patient and her family. We hope this experience could be used as a future reference in similar cases.
