In mid-May 2021, the impact from the spread of the coronavirus pneumonia (COVID-19) outbreak in Taiwan meant the cancer patients undergoing radiation therapy not only had to deal with the cumulative radiation dose but also face the fear of being infected with Covid-19 during the hospital admission for treatment. The risk of infection is a dilemma and real concern for the patients. Radiation oncologists should find a middle ground between the risk of virus exposure and the benefits of treatment. Several consensus and recommendations discussed the issue of telemedicine and avoidance, deferment, and shortening of radiation therapy based on the risk stratification and clinical scenarios. These policies could reduce treatment duration and minimize infection risk during a pandemic.
