癌症是重要的公共衛生議題,其發病率和死亡率逐年攀升。傳統治療如手術、放射線和化學治療仍受限,特別是轉移性或復發性癌症效果不彰。但在過去十年,癌症分子醫學和免疫腫瘤學的進步為精準醫學帶來希望,透過標靶和免疫療法,可針對特定癌症類型或致病機制進行精準操控,結合化療、放療等提高療效,將癌症轉化為慢性病,控制癌細胞生長來維持生活品質。隨著醫療科技發展與進步,癌症治療技術不斷求新求變,推廣精準-個人化免疫細胞治療,然而,我國的免疫細胞治療尚處啟蒙時期,其角色地位有待逐步紮根,國人對治療相關醫療資訊欠缺,甚至持有「自費的效果比較好」之迷思。鑒於免疫細胞治療屬高自費項目,臺灣健保未予任何補助,加上免疫細胞治療會合併其他癌症治療使用,進而產生特定治療未被滿足的照護問題。在全球照護體系中,護理人員提供80%的醫療照護,護理專業也需因應醫療與健康照護生態的轉化,進而掌握與開拓專屬自體細胞治療的護理照護,方能體現護理核心價值。

英文摘要

Cancer, with its incidence and mortality rates steadily increasing, is one of the main public health issues. Traditional treatments such as surgery, radiation, and chemotherapy remain limited, especially in the case of metastatic or recurrent cancers where the treatment effectiveness is limited. However, in the past decade, advancements in cancer molecular medicine and immuno-oncology have brought hope to precision medicine. Through targeted therapies and immune cell therapies, specific cancer types or pathogenic mechanisms can be precisely controlled. By combining these approaches with chemotherapy and radiation therapy, treatment efficacy can be improved, transforming cancer into a chronic disease, and maintaining a good quality of life by controlling the growth of cancer cells. With the continuous development and advancement of medical technology, cancer treatment techniques are constantly evolving, and precise and personalized immune cell therapy has emerged as the new mainstream approach. However, in our country, immune cell therapy is still in its early stages, and its role and status need to be gradually established. There is a lack of public awareness and understanding about this treatment, leading to misconceptions such as the belief that "self-paid treatment yields better results." It is important to note that immune cell therapy is a high-cost treatment that is not covered by the National Health Insurance in Taiwan. Furthermore, immune cell therapy is often combined with other cancer treatments, which creates specific nursing needs that must be addressed. In the global healthcare system, nurses provide 80% of medical care, underscoring their crucial role. In order to uphold the core value of nursing, nursing professionals must adapt to the changes in the healthcare system and develop specialized nursing care for autologous cell therapy. This requires a comprehensive understanding of the treatment, its associated challenges, and the ability to provide personalized care to patients undergoing immune cell therapy.
