運用Watson理論照顧一位無望感的血液透析病患之護理經驗

Nursing Care of a Terminal Patient Undergoing Hemodialysis Treatment under the Watson Theory

本篇個案報告乃運用Watson人性化關懷照顧理論，探討一位面臨無法接受腎臟移植之血液透析病患所產生的無望感，以期能協助病患找回希望。護理期間為92年9月4日至92年10月2日，經評估發現個案呈現問題有1．無望感2．體液容積過量3．無效性保護能力4．便秘。藉由Watson理論，提供十項關懷照顧措施，使得個案能接受無法腎臟移植的事實，重新找回生命中的希望，以健康態度面臨長期血液透析治療，並進一步能增進其生活品質。

This case report discusses the terminal situation of a hemodialysis patient who is ineligible for renal transplantation. It uses Watson's theory of human science and human care to help the patient develop and maintain a sense of hope. The duration of nursing care for this patient was from September 4, 2003 to October 2, 2003. After our evaluation, we found that the patient suffered from: 1. a feeling of hopelessness, 2. hypervolemia, 3. ineffective protecting ability, and 4. constipation. Guided by Watson's theory, we provided ten items of human care to assist this patient accept the fact that he could not receive a renal transplant. We hope that the patient will regain hope, face long-term hemodialysis with a healthy attitude, and further increase his quality of life.
