陪伴生命末期病人及家屬居家安寧療護善終之經驗－個案報告

本篇在探討疾病末期病人及家屬在接受居家安寧照護期間所面臨臨終照護相關的問題，照護個案期間，安寧照護團隊藉由觀察、會談，運用分析瀕死病人對家庭產生的衝擊進行評估，確立病人家屬有對病人身體心像及預期性哀傷等問題。在居家安寧照護過程中，筆者以同理心、尊重的態度與病人家屬建立信任感及良好的醫療諮詢性人際關係，提供持續性之陪伴及支持，協助病人家屬能緩解身、心、靈的不安，並主動提供病人家屬有關臨終的照護及病人死亡過程的相關注意事項，並盡量使其在病人有限的生命中，能夠維持有品質的居家生活。期望此陪伴經驗能與臨床安寧照護團隊一起分享。

We explored the related problems encountered by terminal patients and family members during hospice palliative care. By observations, communications and analysis, our hospice care team evaluated the impact of a dying patient to family. We confirmed the issues of body image and anticipatory mourning on the patient arisen in family members. During the hospice home care period, the author established the relationship with family with empathy and respect, and provided company and holistic support consistently. Meanwhile, when the family members suffered from physical, psychological and spiritual distress, we advised them on terminal care and issues to be concerned in the dying process, so that the patient maintained a quality home living in a confined life. We hope to share this caring experience with other hospice palliative care teams.
