運用Watson關懷理論於肘下截肢個案之護理經驗

Applying Watson's Nursing Theory to Care a Patient with Amputation below the Right Elbow

本文為照顧一位接受右手肘下截肢手術後，導致身體功能改變、自我照顧能力缺失而加重個案的無望感之護理經驗。護理期間由102年2月26日至102年3月23日，以整體性護理評估為指引，經由實際參與照護、身體評估、會談、觀察及查閱病歷等方式進行資料收集，確立其護理問題有疼痛、自我照顧能力缺失及無望感等健康問題。應用Watson所提出的十個關懷照護因素來作為照護病患的指引，建立信任的護病關係，運用關懷的態度予情緒上的協助，提升個案的希望感，引導重建生活價值觀，以樂觀的態度面對截肢後的生活；亦透過持續性關心，鼓勵個案表達內心感受，協助尋求家人的支持，減低心裡創傷與痛苦，改善自我照顧能力，使其重拾信心並發展正向的調適技巧及態度。希望藉此護理經驗能提供護理人員之參考。

This article was a hopeless nursing experience about a self-care deficit aggravated case that resulting in change in physical function after caring for a right elbow. The nursing period was from February 26, 2013 to March 23, 2013. General nursing assessment was used to collect information, psychical assessment, through actual participation care, physical assessment, interview, observation, and search chart. Identified problems included pain, deficiency in self-care ability and hopelessness. This nursing process used Watson proposed ten applications of caring factor as guidelines, to build patient relationship trust, caring attitude to assist in the use of emotional, enhance the sense of hope that the case, values guide to rebuild their lives, face life with optimism after amputation; Also through the ongoing care, to encourage expression of feelings, seeking family support, to reduce pain and trauma of heart, improve self-care ability, to regain confidence and develop positive adjustable skills and attitudes. We hope to share the nursing experience, nursing personnel care as useful reference.
