癌末病患居家照顧工作量及困難度與照顧壓力負荷之相關性探討

本研究以橫斷式相關性研究，探討癌末病患居家照顧工作量、困難度與照顧壓力負荷程度及其相關因素。立意取樣110位癌末病患之居家照顧者，以問卷收集資料，問卷內容包括功能狀態評分表、症狀困擾量表、照顧工作量表及照顧負荷量表。本研究結果發現：就醫協助之工作量最多，提供疾病相關訊息困難度最高，而心理層面之壓力負荷最大。多元迴歸分析，病患依賴程度、症狀困擾程度、照顧者教育程度及每日平均照顧時數可解釋53.5%的照顧工作量變異量；而病患依賴程度、症狀困擾程度及照顧者年齡可解釋23.7%的工作困難度變異量；另照顧工作困難度、照顧工作量及照顧時間，可解釋55%的照顧壓力負荷變異量。此研究結果，利於護理人員了解居家照顧者之照顧經驗，未來研究中可進一步發展改善照顧困難度之護理措施。

The purpose of this cross-sectional correlational study was to explore caregiving tasks, difficulties, caregiver burden, and related factors for home care of terminal cancer patients. 110 home caregivers for terminal cancer patients were selected from a southern Taiwan medical center. Measures used include Eastern Cooperative Oncology Group (ECOG) Performance status, Barthel Index, Symptom Distress Scale, the caregiving tasks scale, and the caregiver burden scale. Result indicated that the most workload was assisting in transportation, the most difficult of caregiving tasks was offering nformation about cancer, and the highest burden was psychological strain. The stepwise regression showed that the level of patient dependency, symptom distress, caregivers' educational levels and caring hours per day could significantly predict the caregiving tasks (R^2=53.5%). And the level of patient dependency, symptom distress and caregivers' age could significantly predict the difficulty of caregiving tasks (R^2=23.7%). The difficulty of caregiving tasks, caregiving tasks and times of care could significantly predict caregiver burden (R^2=55%). Results of this study suggested that the healthcare professionals could develop ways to improve the caregiving difficulty situation and decrease caregivers' stresses.
