患有末期癌症的病人,因其特殊的身體狀況,當其欲搭乘飛機出國旅行時,面對空中飛行的情境充滿了一系列複雜變數,其中包括了氣壓、氧氣濃度及濕度的變化、長時間處於減少身體活動的狀態等等,都充滿不確定性。本文病人在癌症末期階段,在安寧團隊照護協助穩定相關末期症狀,但其有心靈上的需求:搭機返回美國加州落葉歸根,因此需進一步考量與規劃。依據此病人的照護經驗為發想,身為提供安寧緩和療護的醫療人員,透過專業評估,我們考量癌末病人可能面臨的身心狀況,包含呼吸衰竭、貧血、血液栓塞、精神疾病/譫妄、近期手術或腸阻塞、腦轉移、淋巴水腫、裝置有導管或造口、嗎啡類止痛藥物的使用等,而後擬定照護計畫,協助末期癌症病人完成臨終心願。

英文摘要

Among terminally-ill cancer patients, there are many things needed consider due to their physical conditions while traveling abroad by airplane. They might face the different complex situations, such as changes in air pressure, oxygen concentration and humidity, and longtime of being in a state of reduced physical activity. There are full of uncertainty. This patient with terminal breast cancer had good symptoms control under hospice/palliative care team management. However, she wished to go back to California, where she grew up, till the end. Finally, we established the care plan and prepared relevant formal medical documents to help the patient to complete her dying wishes. Based on the experience of this patient, we discussed the specific physical conditions that the terminal cancer patients might encounter, including the risk of respiratory failure, anemia, thromboembolism, mental illness /delirium, recent surgery or intestinal obstruction, brain metastasis, lymphedema, device with catheter or ostomy, the use of opioids analgesics, etc. Via professional assessment and assistance by hospice/palliative care team, holistic care could be carried out.
