疾病末期病人因面臨死亡的特性,其個別的心理動態差異大。因此,要如何運用安全且能協助病人自我覺察及啟動心靈對談的治療性語言,與病人溝通且建立關係,是安寧醫療人員的重要訓練過程。同理心是助人工作中被視為必要的基本態度及能力,醫療人員運用同理心之溝通技巧於末期病人,可促進醫/護病關係之建立,以開啟靈性對話之契機。為協助醫療人員同理心之運用,彰化基督教醫院安寧病房在2018年12月27日到2019年6月27日,共進行六次同理心演練課程,以提升醫療人員之同理心素質,培養同理心運用的態度、知識與技術層面之能力。透過同理心演練課程幫助醫療人員了解:助人者需要敏感且有意願進入病人的內在世界,並了解其想法、感覺及困境,真誠的關懷與同在,不做任何評斷地進入對方的生命經驗裡。此外,醫療人員能運用所學且具體而清楚地回應病人的語意及情感交流,適當調整並修正同理的深度與方向,幫助病人願意敞開內心分享生命歷程,而提供良好社會、心理、靈性照護。

英文摘要

Because of the characteristic of facing death in the end-stage patients, the individual psychological dynamic is very different for each person. It's an important training process for hospice care professionals' to communicate and build relationships with patients, use the therapeutic language for assisting the patient in self-awareness and initiate psychologic talking. Empathy is the basic attitude and ability deemed necessary in helping others. Professionals using empathetic communication skills for end-stage patients could establish a doctor/nurse-patient relationship and thus have opportunities for spiritual dialogue. Therefore, in order to assist the professionals using empathy, we had arranged an empathy program in the Hospice Ward of Changhua Christian Hospital. The empathy training program had been conducted for 6 times since December 27, 2018 to Jane 27, 2019. This program aimed at cultivating the quality of empathy of our professionals and promoting the ability of attitude, knowledge and skill of empathy. After practicing of this course, the following benefits were observed: hospice care professionals needed to be sensitive and willing to enter the patient's inner world, understand their thoughts, feelings and dilemmas, sincere care and presence, and live temporarily without making any judgments. Through empathy training program, professionals could respond specific patients' expression of language and emotion, and then appropriately adjusted and modified the depth and direction of empathy, and helped patients be willing to open their minds to share their life experience. Our professionals can provide better quality of psychosocial-spiritual care.
