不孕婦女深受不孕的困擾,在接受治療的過程,醫療情境更加大其壓力。醫療人員應該協助不孕婦女在求診治療中,減緩壓力。本研究目的爲探討不孕婦女對“自我放鬆訓練”的學習與放鬆狀況。本研究於一所2000床之醫學中心進行,研究對象爲決定接受生殖科技治療之不孕婦女,以立意取樣,共收集17人。研究工具爲壓力自評量表及半結構會談指引。資料收集於不孕婦女接受生殖科技治療前,徵求個案的同意,於一安靜的房間進行韓瑞克森放鬆訓練指導,於結束後,先請個案自評壓力程度,接著進行面對面訪談,且以文字記錄,並以內容分析此質性資料,而量性資料以百分比、平均值、標準差及Kruskal-Wallis Test分析。 研究結果:研究對象學習“自我放鬆訓練”的接受型態,由資料歸類爲三種類型;拒絕學習、強迫自己接受學習、容易接受學習。壓力自評以0爲極度放鬆,100爲極度緊張評估;在拒絕學習之個案,壓力評分平均爲66.3±18.9分,強迫自己接受學習之個案,平均爲60.0±14.1分,容易接受學習之個案,平均爲22.3±23.6分。以Kruskal-Wallis Test檢測,p值爲.014,再以多重比較,顯示「拒絕型」個案之壓力較「容易接受型」之壓力高,具有顯著性的差異。正向的冥想內容有助其接受“自我放鬆訓練”,但負向的冥想內容卻是造成干擾的因素。本結果可提供護理人員在設計有效性的護理處置時之依據。

英文摘要

The purpose of this study was to explore how the infertile women learning ”self-relaxation training”, and to evaluate the stress relaxation status. The research was conducted in a 2000-bed medical center. By purposive sampling, 17 infertile women were recruited. The research instruments included visual analogue scale to understand the subjective feeling of self-relaxation and a semi-structured interview guide. After the subjects finished ”self-relaxation training”, they were asked to self evaluate their own relaxation scale, and then to have a face-to-face interview. The results showed that the acceptance patterns of subjects could be classified as three self-relaxation training groups: (1) reject to accept (23.5%), (2) to accept forcefully (11.8%), and (3) accept easily (64.8%). Basis on a stress self-evaluation scale, where 0 was extremely relaxed, and 100 was extremely tense, the scores in the group of rejecting to accept were 66.3±18.9, the scores in the group of accepting forcefully were 60.0±14.1, and the scores in the group of accepting easily were 22.3±23.6. When the Kruskal-Wallis test and multiple comparisons were used to evaluate the stress score, the scores in the group of rejecting to accept were significantly higher than the scores in the group of accept easily (p=0.014). The positive meditation could improve the subjects to accept the self-relaxation training, while the negative meditation showed the negative effect. The research findings can be a valuable reference for the nurse to design an effective nursing intervention for the infertile women.
