2021年5月中旬,新冠肺炎(COVID-19)大流行在台灣爆發。護理實習生在醫院照顧病人過程中常感經歷恐懼、壓力和焦慮的情緒,擔心自己在實習期間感染病毒、生病或甚至死亡,也害怕傳染疾病給家人。因此,瞭解護理實習生在面臨的新興傳染病時之壓力,進而提供護理實習生適當且具體的正向支持。本研究的目的為調查COVID-19疫情期間,護理系學生於實習時之壓力。本研究採用量性研究方法及橫斷式問卷調查法,以立意取樣方式招募於亞東科技大學至醫院實習的護理系學生進行問卷調查,問卷包含基本資料及護理實習生面對COVID-19流行期間感受到的壓力問卷。本研究以「COVID-19疫情下護理人員的壓力問卷」來評估護理學生於COVID-19流行期間的壓力程度,本量表共分五個構面,平均分數越高代表此構面導致的壓力越大,各構面分析如下:「防護裝備引起的不適」構面得分最高,其次依序為「政策面」、「擔心與社會隔離」、「照顧病人的負擔」,分數最低為「感染控制的困難與焦慮」。以單項來看,分數最高為擔心親友被自己感染(1.65±0.95)、其次擔心缺乏照顧的人力,導致工作負荷過重(1.51±1.00),此兩項皆屬於擔心與社會隔離及政策面的相關;壓力感受分數較低的依序為親友不支持,如被要求拒絕照顧病人或辭職(0.71±0.88)。本研究整體壓力總分為49.47±26.61,為中低壓力。其中以「防護裝備引起的不適」構面壓力得分最高;壓力分數最低為「感染控制的困難與焦慮」。期望此研究結果可為未來護理學生實習時面臨新興傳染病所造成壓力之參考,以便能提供更適切的因應措施。

英文摘要

Background: In mid-May 2021, the COVID-19 pandemic broke out in Taiwan. Trainee nurses caring for patients in hospitals often experience emotions of fear, stress, and anxiety. They worry about contracting the virus, becoming infected or even dying during their internship. Additionally, they were anxious about spreading the disease to their families. Understanding the emerging stress faced by nursing trainees during infectious disease outbreaks is crucial for providing appropriate and specific proactive support to these students. Purposes: The aim of this study was to investigate nursing students' stress in clinical practice during the COVID-19 pandemic. Methodology: This study adopted a quantitative research approach and a cross-sectional survey method. Nursing students enrolled at the Asia Eastern University of Science and Technology were surveyed using a purposive sampling method. The questionnaire included basic information and a survey on the stress experienced by nursing trainees during the COVID-19 pandemic. Results: The study used the "COVID-19 Stress Questionnaire for Healthcare Workers" to assess nursing students' stress levels during the COVID-19 pandemic. The questionnaire consists of five dimensions. Higher average scores indicate greater stress caused by each dimension. The analysis of each aspect revealed that "discomfort from protective equipment" had the highest score, followed by "policy issues", "concerns about social isolation", and "burden of patient care". The lowest score was for "difficulty and anxiety in infection control." Among the individual items, the highest scores were for concerns about infecting family members (1.65 ± 0.95) and insufficient staffing leading to excessive workloads (1.51 ± 1.00). Both items relate to concerns about social isolation and policy issues. Items with lower stress scores included concerns about lack of support from family and friends, such as being asked to refuse care for patients or resign (0.71 ± 0.88). Conclusion: This research used the "COVID-19 Stress Questionnaire for Healthcare Workers" to investigate the stress experienced by nursing interns during the COVID-19 pandemic. The overall stress score was 49.47 ± 26.61, indicating moderate to low stress. The dimension causing the highest stress was "discomfort from protective equipment," while the lowest stress score was "difficulty and anxiety in infection control." Hopefully, these findings will inform future nursing students facing the pressure of emerging infectious diseases during their internships so that appropriate responses can be provided.
