目的：重度創傷性腦傷（traumatic brain injury, TBI）患者因神經心理功能多重受損，重返生涯之路十分艱辛。本個案報告以一名重度TBI患者為例，探討以整合性神經心理治療的復健計畫對於神經心理功能到複雜的日常生活適應之介入成效。方法：個案是名33歲單身男性，工作時從高處跌落致使腦部重創，包括雙側額葉腦挫傷、硬腦膜下腔出血，以及蛛網膜下腔出血。神經心理衡鑑結果顯示患者具有高階神經心理功能受損、情緒困擾，以及疾病適應問題。本案以整合性神經心理治療先確立個別化的短中長期目標，共進行49次療程，並以三次神經心理衡鑑資料進行基準點、介入後，以及一年後追蹤之成效分析。結果：個案神經心理功能、情緒狀態，以及日常生活適應表現三層次指標在介入後皆有進步，並順利全職復工，且一年後功能穩定維持。結論：本案例顯示整合性神經心理治療，可對重度TBI患者達到類化且持續之療效。
英文摘要

Objectives: Severe traumatic brain injury (TBI) patients have a challenging road to returning to their careers due to multiple impairments in their mental functions. In this case report, a patient with severe TBI is used as an example to discuss the effect of comprehensive-holistic neuropsychotherapy from cognitive function to daily living adjustment. Methods: The case is about a 33-year-old male, single, who fell from a height while at work and caused severe brain trauma, including bilateral frontal contusions, subdural hemorrhage, and subarachnoid hemorrhage. The neuropsychological assessment results showed that the patient's higher-order cognitive function was impaired. In addition, due to economic difficulties and insufficient family support, the case also presented emotional and disease adaptation problems. Based on the above assessment results, individualized short-, medium- and long-term comprehensive neuropsychological treatment goals were formulated, and a total of three levels of neuropsychological assessments were conducted to analyze changes in the baseline, post-intervention, and one-year follow-up. Results: After the comprehensive-holistic neuropsychotherapy, the three-level indicators of neuropsychological function, emotional state, and daily life adaptation performance of the patients all improved, and they returned to work full-time, and the follow-up results showed that the intervention effect was maintained stably. Conclusions: This case shows that comprehensive-holistic neuropsychotherapy can achieve generalized and sustained efficacy in patients with severe TBI.
