工業放射線照相是目前工業界常使用的一種非破壞性檢驗方法,本文針對1999年初於南台灣發生的一件工業放射照相所造成的銥-192輻射意外進行探討,主要探討的對象為兩名在此事件中受到曝露的操作人員(以A及B稱之),兩人於近距離處受到活度63居里之銥-192射源近距離的急性曝露約3小時,其中A受到嚴重的輻射傷害,包括手指組織壞死及角質化、染色體異常、血液中血球數減少、甲狀腺瀰漫性腫大以及水晶體出現班點混濁等輻射效應,並於1999年8月進行截肢。本文主要是針對兩人之全身及乳線、骨髓、甲狀腺、眼球、生殖器官等器官所接受之輻射劑量進行評估並探討其目前之身體狀況。評估結果顯示A左手食指於此次意外中受到約40-110西弗(Sv),拇指則約受到11-32 Sv的暴露劑量,眼球約受到0.5-1.5 Sv的劑量,性腺則約受到1.0-3.1 Sv的劑量,其全身有效劑量約為0.8-2.4 Sv。而B左手手指於此次意外中受到約19-63 Sv的劑量,拇指則約受到5.4-19.4 Sv的暴露劑量,眼球約受到0.4-0.9 Sv的劑量,性腺則約受到0.6-1.7 Sv的劑量,其全身有效劑量約為0.5-1.4 Sv。此意外顯示國內對於工業用放射照相非破壞檢驗的輻射安全管制出現極大的漏洞,兩人並無相關操作執照及接受輻射相關教育,也不知劑量配章的重要性,意外事故後兩人也沒有接受專業之劑量評估及醫務監護,在游離輻射防護法實行之際,應加強落實此類工作人員的輻射安全教育及證照制度,此外,追蹤兩人後續的致癌效應及遺傳效應是接下來必須研究的課題。

英文摘要

The industrial radiography is used for non-destructive testing frequently. This study discussed a radiation injury from acute exposure to an iridium-192 that was used for industrial radiography in the south of Taiwan. Two workers (We name them as A and B) were accidentally exposed for an estimated 3 hours from an iridium-192 source that the activity was 63 Curies. One of them developed radiation injury seriously, including necrosis and keratosis of fingers, chromosomal abnormality, decreased blood cell count, diffuse goiter and appearance of dots and opacities in the lens. He underwent amputation of first finger of left hand in August 1999. This study is mainly to evaluate the exposure dose of breast, spine, thyroid gland, lens, testis and whole body of two victims. The result shows that the estimated dose of A resulting from this accident were 40-110 Sv to first finger and 11-32 Sv to thumb of left, 0.5-1.5 Sv to lens, 1.0-3.0 Sv to testis and 0.8-2.4 Sv to whole body. The estimated dose of B were 19-63 Sv to first finger and 5.4-19.4 Sv to thumb of left, 0.4-0.9 Sv to lens, 0.6-1.7 Sv to testis and the equivalent dose of whole body were 0.5-1.4 Sv. The accident shows that the control of radiation safety for industrial radiography appears a large leak. Two victims didn't have any license and didn't train for radiation safety about job. They also did not know the importance of dose badge. There were also not any dose evaluation professionally for the victims and they were not under the tutelage of medical matters. It is necessary to notice the follow-up stochastic effects that are including cancer development and hereditary variation of two workers.
