由於對影像品質的需求提升,放射攝影的重照率往往很高,導致患者吸收更高的輻射劑量。本次希望能透過品管圈的方式,以減少重照率,降低患者輻射劑量。本研究使用品管圈的手法,先識別出問題,並以5W2H方式進行改善前現狀數據蒐集,之後由圈員執行要因圈選並經過真因驗證後分出三大對策群組實施,以改善問題。對策介入後,改善之前重照率為7.4%改善後降至4.0%。同時延伸出附加效益分別為同事的滿意度從81.5%增加到92.3%,並且共在病人身上減少3.1毫西弗的有效劑量。使用品管圈的技術來發現問題和執行對策,並且修改標準操作程序,以降低重照率、減少患者的輻射劑量和醫療不便。同時這也能讓團隊成員們瞭解相互合作和持續思考改善的重要性。

英文摘要

Due to the increasing demand for image quality, the retake rate in radiographic imaging is often high, leading to higher radiation doses absorbed by patients. This study aims to reduce the retake rate and lower patient radiation doses through the use of Quality Control Circle (QCC). This study uses QCC methods to first identify problems and collect data on the current status before improvements using the 5W2H approach. Team members then perform cause selection and verify the true causes, followed by the implementation of three major countermeasure groups to address the issues. After the intervention of countermeasures, the retake rate improved from 7.4% to 4.0%. Additionally, there were ancillary benefits, including an increase in colleague satisfaction from 81.5% to 92.3% and a reduction of 3.1 mSv in effective dose to patients. Using QCC techniques to identify problems and implement countermeasures, as well as modifying standard operating procedures, can reduce the retake rate, decrease patient radiation doses, and minimize medical inconvenience. This also helps team members understand the importance of mutual cooperation and continuous improvement.
