本研究目的旨在探討不同運動訓練強度對大鼠阿基里斯肌腱斷裂後修復的影響。以36隻8週大的Spraque-Dawley公鼠為對象,隨機分成控制組、低強度運動組、中強度運動組,運動組介入12週不同運動強度訓練後,將所有大鼠右後肢阿基里斯肌腱切斷並自行在籠內修復,評估受傷前(運動介入後肌腱切斷前)、傷後5天及4週的阿基里斯肌腱功能指數,犧牲後各組隨機取3隻觀察肌腱膠原蛋白生長情形,其餘9隻進行肌腱機械特性測試。結果:一、受傷後5天運動組的AFI(Achilles tendon Functional Index)值顯著比控制組高(中強度組:p=.024,低強度組:p=.006),傷後4週3組間則無顯著差異。二、膠原纖維結構中3組均出現波紋結構,但中強度運動組的排列較紊亂,膠原蛋白種類比例(collagen type proportion Rate, CTR)方面均無顯著差異。三、傷後4週3組的受傷肢肌腱直徑(Achilles Tendon Diameter, ATD)都顯著大於正常肢(控制組:p=.000,低強度組:p=.000,中強度組:p=.000)。四、低強度組的受傷肢肌腱斷裂強度(Breaking Stress,, BS)顯著大於正常肢(p=.005),而中強度運動組的受傷肢小於正常肢。五、受傷肢與正常肢之間的肌腱拉伸應力(Tensile Stress, TS)達顯著差異(控制組:p=.001,低強度組:p=.002,中強度組:p=.000)。本研究發現:中、低強度的運動訓練會使受傷後第5天的步態恢復較快,但受傷後第4週時中強度運動組的受傷組織斷裂強度開始變差,反之,低強度組的斷裂強度在受傷後第4週有較好的趨勢。

英文摘要

The aim of this study was to investigate the effect of exercise training pre-injury on Achilles tendon rupture repair in rats. Methods: Thirty-six Spraque-Dawley (SD) male rats (eight weeks old) randomly assigned to three groups: control, low-intensity exercise, medium-intensity exercise. Intervention with different exercise intensity to both two exercise groups at twelve-week. After that disconnection right hind limb Achilles tendon to all rats. Without any exercise rehabilitation prescription to all rats and self-healing after injury. The Achilles tendon functional index (AFI) were evaluated at pre-injury, five-day and four-week post injury. Each groups were randomly sacrificed three rats and observation growth of collagen to Achilles tendon. Each group were sacrificed nine rats and test for diameter, breaking strength, tensile stress of tendon at four-week post injury. Result: AFI values: No found significantly between the three groups at pre-injury and four-week post injury, however, both two exercise groups were significantly better than control group of AFI at five-day post injury. Collagen growth: All group was form crimp to collagen structure, but medium-intensity exercise group was confusion than both other two groups of collagen structure at four-week post injury. No found significantly between the three groups of collagen type proportion at four-week post injury. Tendon diameter: No found significantly to normal limbs and injury limbs to tendon diameter between the three groups at four-week post injury, but normal limbs were significantly smaller than injury limb to tendon diameter in each groups. Tendon breaking strength: Not found significantly to normal limbs and injury limbs to tendon breaking strength between the three groups, but normal limbs were significantly smaller than injury limbs to tendon breaking strength of low-intensity exercise group at four-week post injury, and injury limbs were smaller than normal limbs to tendon breaking strength of medium-intensity exercise group at four-week post injury. Tendon tensile stress: No found significantly to normal limbs and injury limbs to tendon tensile stress between the three groups, but injury limbs were significantly smaller than normal limb to tendon tensile stress in each groups. Conclusion: Although the walk gesture recovered faster at five-day post injury in medium- and low-exercise intensities, the breaking strength in injured tissue in the medium exercise intensity group become worse at the 4^(th) week post injury. On the contrary, the breaking strength in the low-exercise intensity tended to be better (higher).
