目的:瞭解醫院護理人員代謝症候群之盛行率和發生率及其年度別、年齡別、性別與工作屬性之差異。方法:採回溯世代研究法;以曾任職北台灣某醫院並參加一次以上健檢之護理人員為研究對象,回溯期間自2013年1月至2016年12月止,參與健檢護理人員共有3654人次(3238人)為盛行率樣本,排除只檢查一次的2916人,共738人次(322人)為發生率樣本。 結果:代謝症候群2013-2016年累積盛行率為7.0%;盛行率以2015最高12.0%;男性明顯高於女性(12.3%>6.7%);年齡別以50~<66歲最高達24.2 %、其次為40~<50歲16.3 %;工作屬性以行政主管19.9%最高,其次為行政人員(13.3%)。整體護理人員(共計322人有兩次以上健檢者)之代謝症候群2013-2016年累積發生率為11.5%,年齡別30~<40歲(14.3%)居首,其次為40~<50歲發生率13.2%,男性高於女性(18.2% > 11.3%);工作屬性以行政人員16.7%最高,其次為普通病房(14.9%)及門診單位(14.0%)。多變項分析顯示年齡別與發生率間具顯著相關。結論:影響醫院護理人員代謝症候群之危險因子為年齡,建議醫院針對30歲以上護理人員提供代謝症候群預防與治療服務。

英文摘要

Purpose. The study investigated the prevalence and incidence of metabolic syndrome among nursing staff in a teaching hospital, as well as its distributions stratified by year, age, gender, and job attributes. Method. Under a retrospective cohort design, the study evaluated nursing staff who had received at least one health examination between Jan 2013 to Dec 2016. A total of 3654 person-time (3238 persons) was recruited from health examination database and only 738 person-time (322 persons) received more than one health examination. Result. The cumulative prevalence of metabolic syndrome from 2013 to 2016 was 7.0% and the yearly prevalence was highest in 2015 (12.0%). Metabolic syndrome was significantly more prevalent in men than women (12.3%>6.7%). In terms of age, the highest prevalence was found between 50~<60 years old (24.2%), followed by 40~<50 years old (16.3%). With regards to job attributes, administrative supervisors had the highest prevalence (19.9%), followed by administrative staff (13.3%). For those who had more than one health examination between 2013 and 2016, the cumulative incidence of metabolic syndrome was 11.5%; the highest incidence was found in the 30~<40 age group (14.3%), followed by 40~<50 (13.2%); and men showed higher incidence than women (18.2%> 11.3%). Multivariate analysis showed that the incidence had a significant positive relationship with age but is not related to seniority, job attributes and gender. Conclusions. We suggest that a prevention and treatment program is needed for nurses above 30 years old.
