Aviation is a relatively safe mode of transportation. After the disastrous 9/11 terrorist attacks, many governments in the world have contributed advanced and progressive efforts in enhancing aviation safety and airport security performance. Despite of the immediate goal of revenue recovering, maintaining a risk-free aviation workforce or environment is believed to be another top priority pertaining to contemporary airline operation. However, aircraft accidents still occur periodically and are, in most part, due to human errors. Therefore, a well-designed safety training aiming to close up the loophole of the existing safety net is needed. With this in mind, the root causes leading to human errors must be identified and eliminated before being transferred into accidents. Following the same research vein, this study was an attempt to prepare a systemic error-elimination model based on a mini-Fault Tree Analysis (FTA) model. Accident causes were categorized. A basic model of aviation FTA is now available for aviation authority and air carriers regarding policymaking and safety training accordingly.
