自從健保於84年開辦以來，醫療院所收入絕大部分來自向健保局申報申請金額，然而醫院之醫療資訊系統(Hospital Information System)複雜程度不雅於金融業之資訊系統，而許多醫院大都忽略資訊系統之重要性，在技術、人力、預算上皆無法得到足夠的支援。軟體系統開發常被關注風險管理議題，但是在主機系統與整體開發技術上的風險管理，卻很少人談及，在本文以AT醫院為例，在幾次的主機更換事件當中，引發許多風險危機上的重大議題，AT醫院有無記取教訓，還是再次沉淪危機當中。

Hospital Information System (HIS) is the backbone for the hospital administration. Especially after the implementation of the National Health Insurance (NHI) in 1995, reimbursement from the Bureau of National Health Insurance has become the major source of hospital's revenue. However, many hospitals weigh very little on their HIS evidenced by insufficient information technical support, manpower and budget allocated to HIS despite a HIS is as complicated as a banking system. Risk management is the most discussed topic in the software system development. On contrary, very few people refer to the risk management in either the HIS computer hardware system or integration for the whole system. Inevitably, the AT Hospital, the case in this paper, seems to face an information crisis as it changed its host computer system each time. Has the AT Hospital learned from each crisis or will it fail to manage the same crisis?
