中醫處方在臨床上是以兩味以上藥物組成，現今使用成方濃縮製劑（科學中藥），常忽略中藥的特性。處方應以中醫理論為原則辨證審因，按照君臣佐使的結構，才能處方嚴謹，君臣有序，佐使分明，開出與證相對應的處方。古書記載，中藥配伍禁忌有一定原則，現今配方處置常被忽略；中醫處方應了解其禁忌。處方時須深入了解古人對人體用藥之經驗及慎用、禁用的法則。現代中藥研究發展對中藥不良反應的認知中，應了解中藥的用藥安全的重要。因此中藥處方需注意到其禁忌及副作用，並且深入了解用藥的禁忌包含病證用藥禁忌、配伍禁忌、妊娠用藥禁忌、中西醫藥合用禁忌及中西用藥安全，才能提升中醫處方的水準。處方用藥，不可有方無法，亦不可有法無方，而是有方有法。

In Chinese Medicine, using the combination of more than two kinds of Chinese herbs is common in clinical treatment. Sometimes, the use of concentrating Chinese herbs ignores the attributes of the herbs. Therefore, Chinese Medical description has to the following directions: identify the illness and find the factors. Follow the structure of monarch and minister and assistant and guide. Following these directions could make the structure of description more complete. It was said that there are principles and contradictions in describing Chinese herbs. However, sometimes they were ignored. The development of Chinese herbs makes us know the adverse drug reaction of them. Therefore, we would know the safe use of Chinese herbs. We have to notice the side effect and contradictions of Chinese herbs, including syndrome, description, pregnancy, combining use of Chinese herbs and Western medicine, and the safe use of Chinese herbs and Western medicine. That way, we could raise the level of Chinese medical description. Therefore, the rule of description has to be obeyed.
