中醫治療創傷性動眼神經麻痺之病例報告

一位76歲女性患者因車禍頭部外傷，造成動眼神經麻痺，以至於左眼臉下重且左眼球往外下方偏移一個月。西醫處理後症狀仍未改善，故尋求中醫治療。經辨證屬頭部氣滯血瘀，瘀血阻滯經絡導致胞目全縱而不收，用通竅活血湯以活血化瘀、通竅活絡，配合針灸治療，處理3個月後症狀獲得改善，藉此病例報告以提出治療經驗，供臨床上參考和運用。

A 76-year-old woman with oculomotor nerve palsy resulting from a traffic accident presented to our Chinese medicine OPO after being treated with western medicine with unsatisfactory results. Physical examination revealed the patient had ptosis of the left eyelid and inferolateral deviation of the left eye. Using traditional Chinese medicine pattern diagnosis, the patient was diagnosed with qi stagnation and blood stasis of the head. Blood stasis caused a block in the main and collateral channels passing through the eye, thereby inducing ptosis of the eyelid. We used acupuncture therapy in combination with the Chinese medicinal formulas Tong Qiao Huo Xue Tang to promote blood circulation, free the orifices and quicken the collateral channel s. After three months of treatment, the patient's symptoms were considerably improved. In the following case report, we present our experiences and findings as a resource for practitioners in their own treatment of oculomotor nerve palsy.
