護理人員對精神病人之社區居家照護服務品質的觀點

本研究主要為了解實際執行社區精神衛生照護服務工作的護理人員，對社區精神病人照護品質的觀點。運用半結構訪談指引進行三次焦點團體及一次個別訪談，收集14位護理人員對社區精神病人的照護品質的觀點。運用內容分析方式，進行資料分析。研究發現社區照護品質包含四個主題：一、服務人員的能力—拓荒者之能力；二、社區服務方法—提供無縫的照護；三、個案的需求面服務—尋找生活意義；四、服務資源聯盟—聯結病人在地生活需求的照護。本成果可提供精神病人社區精神衛生照護品質指標建立，界定社區精神衛生照護服務作業規範、人員培訓規劃，以及作為衛生主管機關，未來推動精神病人社區精神衛生照護品質評鑑之參考。

The aim of this study is to describe how the psychiatric homecare nurses perceive nursing quality while engaging in psychiatric care in the community. The study has involved with three focused groups, one time sophisticated interview, and as well as a semi-structured approach to help understand the issues of temporary care with qualify in the community. The method of content analysis was adopted to run the quality data where fourteen nurses were in charge of interviews. We have found that four descriptive themes emerged: 1. the staff's ability of rendering service, also the pioneer's competence for discovery, 2. the method of community service, i.e. providing patients with coherent care, 3. the service of satisfying patients' needs, such as search for the meanings of life, and 4. the service of resource sharing, to link patients' needs with health care networks. It is important for nursing staff to demonstrate psychiatric care with quality and to continue evaluation on care. We believe that the results of this study could be used as a reference for health care accreditation. It could be used as well as guidance for providing psychiatric community care, developing the mental health policies and regulations along with health care services which are provided to psychiatric patients in the community.
