運用賦能概念協助一位思覺失調症個案之護理經驗

本文是運用賦能（empowerment）概念協助一位思覺失調症個案促進服藥動機，提高對治療的遵從性及學習疾病自我管理之護理經驗。照護期間為2014年4月24日至2014年7月10日。筆者藉由觀察及會談進行整體性護理評估，歸納主要護理問題有：一、感覺及知覺紊亂；二、知識缺失；並以真誠、接納、關懷及耐心的說明，與其建立穩定治療性的夥伴關係，並運用賦能概念擬定符合個別性需求之護理計畫。透過此護理過程，教導個案自我察覺使用幻聽自助技巧之因應策略，能有效減少幻聽干擾頻率（由17次／週減少為5次／週）及改善焦慮及害怕等情緒；並在探索自我疾病與藥物治療歷程中，引發個案投入疾病自我管理的學習，併行疾病與藥物相關認知的問卷調查，其施測結果由前測40分提高為100分。顯示個案能正向的肯定自我，增強疾病與藥物治療之認知，並建立對疾病與藥物治療的決策能力，因而願意接受藥物治療，最終能順利回歸社區，提升個案及家人之生活品質。

This paper is about an experience of using the concept of empowerment to help a patient suffering from schizophrenia, and to promote the patient’s motivation of taking medication to be in compliance with treatment and self-learning. The nursing period was from April 23 to July 10, 2014. According to our observation and interview with the patient, the major nursing problems are: 1. sensation and perception confusion, and 2. a lack of knowledge. Through the means of treatment partnership and using the concept of empowerment to design the nursing plan, the patient’s condition was improved, the interference of auditory hallucination was decreased (i.e.17/week decreased to 5/week), and had a good improvement in emotion. The questionnaire of self-learning and in compliance with medication was enhanced from score 40(pretest) to score 100.
