本報告係照護一位糖尿病病足截肢患者的護理經臉，筆者於96年9月6日至96年9月24日住院期間，連用Gordon十一項功能性健康型態評估工具，以會談，觀察，傾聽等技巧，收集資料加以整理分析後，確認個案截肢後有1.知試缺失；2、悉性疼痛；3、身體心像紊亂；4、身體活動功能障礙等護理問題。在護理過程中，運用主動關懷、同理及接受等支持技巧，針對個案間題提供個別性護理計畫，並選擇直接的身體護理與個案建立良好護病關係，協助個案度過截肢後所面臨生理、心理及社會之衝擊。使其身心社會得到正向的適應。冀望此護理經驗做為讓理同仁日後照護此類病患之參考。

This case report describes the care of a diabetes patient with a foot ulcer. The nursing period was from Sep 6, 2007 to Sep 24, 2007 and the nurse performed conversations and observations in order to collect data. By applying the eleven functional health patterns as described by Marjory Gordon, it was found that the nursing assessment clarified certain nursing issues including deficiency of knowledge, acute pain, disturbed body image and impaired physical mobility. In the proves of nursing this patient, the author used nursing skills including active caring, compassion and acceptation. The author provided individual ewe planning for patient and established a good nurse-patient relationship. We helped the patient to pass through physical, mental and social impacts and to make a positive adaptation. We hope this article provide good references for caring a Diabetes Patient with Foot Ulcer.
