本篇個案報告主要描述一位復發性肺栓塞患者之護理經驗，照護期間藉由Gordon十一項健康功能型態為評估架構，運用直接照護、觀察、會談及與家屬溝通等技巧收集資料，分析歸納出心肺組織灌流改變、急性疼痛、焦慮及高危險性損傷等四個主要護理問題，筆者透過護理活動監測心肺灌注不良之症狀，協助維持生理功能及與家人提供心理支持，採傾聽、主動關懷並維持身體舒適，與個案建立良好的治療性關係，深入瞭解其內心的衝擊與壓力，提供預防肺栓塞再次復發相關的衛教，使個案栓塞情形改善並恢復健康，順利轉出加護病房。期能藉由本文提供護理人員於臨床上照顧復發性肺栓塞患者之參考，並增進護理人員對肺栓塞之認知，進而提昇此類患者之護理品質。

This case study reported the nursing care for the patient with recurrent pulmonary embolism. During the patient stay in the intensive care unit, the authors used Gordon 11-item Functional Health Pattern as assessment Framework, the use of direct care, observation, talks and communication with their families and other techniques to collect data, and generalized four nursing problems, including tissue perfusion altered cardiopulmonary, acute pain, anxiety, and risk for injury. In the process of nursing care, problems and nursing principal of caring recurrent pulmonary embolism were identified.
