一位多重性創傷病人之護理經驗

A Nursing Experience of a Patient with Multiple Trauma

本文是協助一位因車禍造成多重性骨折且面對多次手術造成生理問題及心理壓力之個案的護理經驗。筆者於2017年6月1日至2017年6月19日照護期間，運用Gordon十一項功能性健康型態進行護理評估，確立個案有急性疼痛、身體活動功能障礙、自我照顧能力缺失、焦慮之健康問題。照護期間筆者運用疼痛相關緩解處置，減輕傷口及移位產生的疼痛，同時與醫療團隊合作，與個案共同訂定個別性復健計畫，改善身體活動功能障礙，提昇自我照顧能力，並藉由早期介入出院準備服務，提供輔具租借服務，與團隊共同參與居家環境規劃及提供心理支持。學習出院後的居家自我照顧與復健運動，強化自我角色功能與價值感，減輕焦慮，如期出院與適應居家自我照顧生活。

This article aims to assist a patient who had multiple fractures caused by an vehicle accident and faced physical problems and psychological stress after several operations. During the nursing period from 1st to 19th June, 2017, the author applied Gordon's 11 functional health pattern to conduct nursing assessments and the result indicated that the patient had health problems relating to acute pain, impaired physical mobility, self-care deficit and anxiety. During the nursing period, the author used related pain-relief treatments to help reduce acute pain caused by wound and movement; on the other hand, the author cooperated with medical care team and discussed with the patient to establish collectively an individual rehabilitation plan for the case patient, with an aim to improve physical mobility and enhance self-care ability. Further, through early intervention of discharge planning, we provided rental service for assistive technology devices, joined the medical care team for home environment planning after discharge and gave psychological support. After studying the home self-care and rehabilitation exercise after discharge, the patient strengthened self-role function and values, his anxiety was reduced. Finally he was discharged as planned and adapted himself to self-care life at home.
