「食不下嚥」一位腦出血併吞嚥障礙之照護經驗

本個案為一位82歲長者因跌倒導致創傷性腦出血，面對個案因身體活動受限和吞嚥障礙，日常活動失去控制產生無力感，印尼籍看護工因文化語言隔閡，對學習照護技巧理解程度有限，引發筆者探討動機，護理期間2020年5月30日至6月19日，以Gordon十一項功能性健康評估工具，進行整體性評估，針對吞嚥障礙、無力感及照顧者角色緊張之健康問題討論，利用護理專業知識整合跨團隊資源，指導吞嚥訓練，回復個案由口進食；共同規劃漸進式復健運動，增強肌力，提升個案自我照顧能力，適應疾病衝擊；提供外語翻譯衛教資訊，協助印尼籍看護工了解照護內容，介紹同鄉友人，交流生活經驗，最後結合長照2.0擬定返家照護計畫與目標，減少印尼籍看護工獨自照顧壓力。建議設計多語衛教影片，歸納常見問題彙總，方便外籍看護工閱讀學習，提升生活照護品質。

This report describes an experience of an 82-year-old elderly with traumatic intracranial hemorrhage caused by fall. The impaired physical mobility and dysphagia of patient and language barriers of Indonesian caregiver caused the author to explore the motivation. The nursing period was from May 30th to June 19th, 2020, by using Gordon 11 functional health assessment, to found that the main health problems, including dysphagia, powerlessness, and caregiver role strain. We integrated cross-team resources with nursing expertise to improve swallowing skill in cases, elevated himself-care through rehabilitation, than used Indonesian language to explain disease information, healthcare skills, introduced compatriots to Indonesian caregiver, and formulated patient’s home-care needs with long-term care 2.0. Through design multilingual health education videos and summarize the common problems make foreign caregivers learn better, thereby improving the patient's quality care.
