本文乃描述一名因車禍而造成足部挫、擦傷之青少女個案，合併有全皮層皮膚缺損必須接受植皮手術之護理經驗。由於個案過去未曾有過手術經驗，且對手術過程不了解及恐懼，遲遲不願接受手術治療而造成身心不適。運用Gordon十一項功能性健康型態評估工具，發現個案的護理問題為皮膚完整性受損、焦慮、睡眠型態紊亂、身體心像紊亂。自2007年7月31日至2007年8月10日照顧期間針對上述護理問題，給予個案完整的護理措施，包括提供心理支持及手術須知，以協助個案勇敢地接受手術治療、學習出院後植皮皮膚的自我照顧，及接受身體外觀的改變。個案在經過適當的手術治療及護理措施後，傷口順利的癒合，也接受身體外觀的改變。希望藉由此次的護理經驗分享，能提供臨床照護青少年之參考。

This article describes the nursing experience of a teenage girl who received skin graft surgery for an abrasion on her foot. The patient initially was reluctant to accept skin graft surgery due to fear of the surgical procedure. The Gordon 11-item functional health pattern for nursing assessment indicated the patient's health problems included impaired tissue integrity, anxiety, sleep pattern disturbance, and body image disturbance. Nursing interventions included providing emotional support, providing appropriate surgical information, teaching post skin graft self-care procedures, and assisting the patient to accept her skin condition. It helped the patient to overcome the fear of performing the operation. The patient's skin graft was successful and she was able to demonstrate acceptance of her post surgical appearance.
