個案管理照護模式可以解決傳統缺乏整合、片段式醫療照護之問題,近年來已被廣泛應用於醫療照護系統,行政院衛生署國民健康局(現為行政院衛生福利部國民健康署)於2010年1月制定「癌症防治醫療機構應建立院內腫瘤個案管理照護制度」,將個案管理照護制度正式納入臺灣癌症照護之一環。本文主要目的為介紹本院建置腫瘤個案管理照護模式之經驗,本院之特色包括:全癌服務、強調臨床照護、24小時電話諮詢,希望能提供國內其他醫療機構發展此一照護模式之參考。個案管理可以增加病人之治療遵從率,進而提升疾病治療成效、促進生活品質,是一個非常值得持續推廣應用之照護模式,目前國內正積極發展,未來可再藉由更有效、更有說服力之個案管理成效實證,以呈現個案管理師與個案管理照護模式之價值。

英文摘要

The purpose of this article is to introduce the experience of establishing the oncology case management model in a hospital in metropolitan Taipei. The case management model is widely used in the health care to fill the gap of a disintegrated traditional health care system. In January 2010, Bureau of Health Promotion, Department of Health, Executive Yuan (current name "Ministry of Health and Welfare") of Taiwan officially adopted the case management model in the cancer care system. This model includes a full scope of cancer services, emphasizing clinical care and 24-hours telephone counseling. Case management may improve patients' compliance to treatment, enhance the disease outcomes, and promote their quality of life. The models of case management are being developed and are worth continual promotion in Taiwan. The effectiveness of case manager and case management will be more evident in the future after more studies are revealed.
