本文目的為描述無縫照護對癌症兒童家庭的意義，並依據文獻與國內現有資源提出四大護理策略：1.將個別教育計劃納入出院準備計劃；2.將轉介支持服務納入出院準備計劃；3.強化兒癌存活者與雇主的雙向溝通；4.建置癌症兒童家庭的健康監測系統。期望本文能增進護理人員與相關專業團隊合作的能力，使癌症兒童家庭於不同的生命階段、疾病過程以及生活場域皆能持續獲得適切的照護。

This article describes the significance of seamless care to family with cancer children and addresses four nurse-led strategies based on literature review and current resources in Taiwan. The four strategies are: 1) embed individualized education plan in the discharge planning, 2) incorporate supportive and referral services in the discharge planning, 3) strengthen bilateral communication between childhood cancer survivors and their employers, and 4) construct a health surveillance system for family with cancer children. These recommendations will require nurses' competences in interdisciplinary collaboration that will provide family of cancer children with appropriate and continuous nursing care across life span, cancer trajectory, and diverse situations of life.
