乳癌位居女性國人癌症發生率第一名，對大多婦女來說是威脅生命的重大壓力事件，雖可經由篩檢早期發現、早期治療，但婦女在面對乳癌篩檢、診斷及治療決策時會有許多負面情緒反應與心理壓力，進而影響其思考與訊息處理能力、降低生活品質。本文分享某醫學中心藉由實證模式建立「實證整合型乳癌照護指引」，整合就醫流程，提供專人導航，和客製化照護，讓乳癌婦女在診斷前期、診斷期、積極治療期及追蹤期各階段都能得到無縫式優質照護，也為其他癌症領航照護建立標竿。

This paper describes the implementing an evidence-based breast care clinical guideline in a medical center in metropolitan Taiwan. Breast cancer is the leading cancer diagnosis for women in Taiwan. The diagnosis can be devastating in one’s life. Early detection of cancer greatly improved the rate of successful treatment. However, the negative emotions and psychological distress during screening, diagnosis and treatment decisions adversely affect thinking and information processing, leading to decreased quality of life. This project established a seamless service model for breast cancer patients that included an integrated examination process, a navigator program and patient-centered care. It begins with the pre-diagnostic period and continues through diagnostic, cancer treatment and five-year follow-up. It is recommended that this seamless cancer patient service model be a benchmark for other cancer navigator programs.
