心臟衰竭是全球性健康問題，高罹病率除了為病人帶來極大的影響，也對家庭、社會、國家帶來沈重的經濟負荷。良好的自我照顧可以緩解病人症狀、減少住院天數和降低再入院率，及改善病人生活品質。然而研究顯示不論國內外心臟衰竭病人的自我照顧能力皆不足，研究證實自我管理計畫對於心臟衰竭病人的自我效能有正面影響。本文藉由綜整心臟衰竭病人自我管理相關研究，提供護理師設計自我管理方案之參考，期能有助國內心臟衰竭病人自我管理之推廣。

This article is a review of research on heart failure self-management programs. The high prevalence of heart failure around the world and its impact on patients, families and society become both a major health and a financial burden. Literature review indicated that most patients diagnosed with heart failure were inadequate in management of their health. Research showed that those patients under a sound self-management program had improved physical symptoms, reduced hospital days and re-admission rate, quality of life, and self-efficacy.
