管路安全為衛生福利部公告2016~2017年醫院醫療品質及病人安全年度目標之一,管路照護不慎可能造成嚴重合併症甚至威脅生命。本單位為神經外科病房,於2013年第2季傑克森引流管(Jackson-Pratt drain, J-P drain)滑脫率高達6‰。分析原因為缺乏護理指導工具、固定引流管裝置及針對護理師之引流管照護流程、稽核制度未建立。專案小組製作中文版及印文版之護理指導單張、拍攝護理指導影片及設計固定引流管裝置、進行教育訓練及稽核等方法,以作為解決方案。方案實施後,2014年第一季滑脫率由6‰降至0‰,護理師照護正確率由70.1%提升至97.3%,病人及照顧者J-P drain照護認知正確率由53.4%提升至78%。此專案手法可有效降低J-P drain滑脫率,提升護理師、病人及照顧者對於管路照護之知能,使病人安全獲得保障。 |
英文摘要 |
Tube safety is one of the goals listed in the 2016-2017 Taiwan Patient Safety Goals for Hospital. Incidents in tube management can lead to severe complications including death in extreme cases. The dislocation rate of Jackson-Pratt drains was 6‰ in a neurosurgery ward in the second quarter 2013. The gaps identified were lack of nursing instruction tools, lack of tube securing device, lack of tube management policy and protocols, and lack of quality assurance process. The ward formed a tube safety team that established bilingual (Chinese and English) instruction guidelines for tube management, produced nursing instruction videos, designed a fixing appliance to secure tubes, and implemented corresponding training and evaluation programs. During the first quarter 2014, the Jackson-Pratt drains dislocation reduced to 0‰, the care efficacy of registered nurses improved from 70.1% to 97.3%, and patients' and caregivers' awareness of the care for Jackson-Pratt drains increased from 53.4% to 78%. The resolutions for Jackson-Pratt drains safety diminish the dislocation rate and enhance the competency of tube management for nurses patients, and caregivers. |
