背景:處理急救事件是應屆畢業護理師進入職場面臨的重大壓力之一,對急救藥物認知不足容易導致給藥錯誤、產生工作挫折,進而影響病人安全及護理人員留任。目的:探討應屆畢業護理師對急救藥物的認知,及其與面對急救事件時壓力感受之相關性。方法:本研究採橫斷式調查法,以北部某醫學中心之應屆畢業新進護理師為研究對象,運用結構式問卷於報到時及工作六週後進行二階段調查,研究期間為2015年6-10月,調查結果以SPSS Window 20.0版套裝軟體進行分析。結果:總計164位護理師參與,急救藥物知識平均得分為46.9 ± 17.1分,曾經接受急救訓練者知識得分顯著高於未曾接受訓練者。六週後有38位護理師已經歷過病人急救事件,其中94.7%對此感受到很大的壓力,且急救藥物知識得分越低者,在經歷急救事件時壓力感受程度越高(p = 0.002)。結論:在護理養成教育及二年期護理師訓練計畫中應規劃系統性課程,提升應屆畢業護理師的急救能力,降低工作壓力。 |
英文摘要 |
Background: Newly graduated nurses face various stresses during their practice, especially in situations involving resuscitation that require sufficient competence. The insufficiency of knowledge about resuscitation medication might result in occupational stress and malpractice. Purpose: The aims of this study were to evaluate newly graduated nurses' knowledge of resuscitation medication and to investigate the association between this knowledge and their stress perception, self-evaluation, and obstacles in administering resuscitation medication. Method: A cross-sectional and correlational study was conducted in a medical center in Northern Taiwan with newly graduated nurses from June to October 2015. The knowledge of resuscitation medication was evaluated using a structured questionnaire after their enrolment and their stress perception was evaluated after 6 weeks of their clinical practice. The collected data was analyzed using SPSS for windows 20.0. Result: A total of 164 newly graduated nurses employed by the medical center were enrolled in the study. The average score on resuscitation medication knowledge was 46 ± 17.1. Nurses who had resuscitation training scored higher on resuscitation medication knowledge than nurses without the training. Only 6.1% of nurses believed that they had sufficient resuscitation medication knowledge and up to 90.2% believed that they lacked training. Six weeks later, 38 nurses encountered emergency resuscitation situations and 94.7% of them experienced tremendous stress. Stress perception was negatively correlated with resuscitation medication knowledge (p = 0.002). Conclusions/Implications for Practice: The occupational stress from instances of emergency resuscitation in newly graduated nurses was correlated with the insufficiency of resuscitation medication knowledge. Pre-employment training may reduce occupational stress and mitigate the risk of malpractice. |
