本文描述一位糖尿病人,因血糖控制不良引發尿毒症病初次進行血液透析之護理經驗。護理期間為2015年4月11日至4月20日,筆者利用每次透析期間,藉由運用觀察、傾聽、會談、及陪伴及身體評估,以Gordon十一項健康功能評估方式收集資料,加以分析整理,確立個案護理問題有:體液容積過量/尿毒症致無法排除水份和鈉離子、高危險性傷害/出血與調適障礙/初次行血液透析;照護期間,筆者提供整體性、連續性護理活動,降低疾病及透析治療對個案之影響,並個案及家屬討論擬定返家後照護計畫,並期望此照護經驗能提供臨床護理人員日後運用於尿毒症並血液透析個案之心理及生理之照護參考。 |
英文摘要 |
This article describes the experience of nursing care of a patient diagnosed with uremia undergoing first hemodialysis due to complications from diabetes mellitus. The care occurred during a 10-day period in 2015. The author adopted Gordon’s 11 Functional Health Patterns as the assessment method, collected information by observation, listening, interview, physical examination and nursing care. The patient had the following aspects that required specific care, including excess body fluid, high risk injury and impaired adjustment. The author provided holistic and continual care to reduce the negative effects from the disease and hemodialysis, and discussed the home-care plan with both patient and family. The author encourages nurses to pay more attention to the physical and mental status of patient with uremia and hemodialysis. |
