目的:瞭解軍護隨隊出勤時,在高擬真戰場壓力環境威脅下對於緊急救護能力成效、身體心理適應參數與建議,以供日後發展軍護隨隊出勤戰術醫療訓練課程與軍陣醫學教育相關政策擬定參考。方法:採用類實驗研究設計,比較軍護人員在經過戰術戰鬥救護訓練前後於高擬真戰場環境下進行緊急醫療能力、生理指標參數之差異。結果:招募12位受試者,經由介入措施後,測驗成績皆有提升。除團隊作業外,在聯絡溝通、資源管理、裝備應用、執行作業標準與現場心智等有顯著差異高。討論與結論:運用高擬真模擬戰場,雖然提升了戰術醫療能力,但發現學員體力負荷不足,亦不適應醫院以外的醫療作業環境,因此未來須強化高擬真訓練課程,因應現況國內無戰時環境,建議未來在軍護隨隊出勤應加強課程訓練深度與簡化傷情,以處理大出血與遠離敵火下傷患救治為主的高擬真教案。

英文摘要

Objective: To understand the nurses' ability to provide critical care nursing and to evaluate the effects of tactical trauma casualty care training program in a simulated combat battlefield. Findings of this study will be used for future expansion of simulated battlefield medicine training program. Methods: A quasi-experimental design with a convenient sampling of 12 nurses. Data were collected from physiological parameters, checklist and questionnaire survey that included critical care ability, stress reaction, adaptation of critical care nursing on simulated battlefield. Results: There was significant improvement in communication, resource management, device manipulation, protocol operation, and mental status post training except team work. Conclusions: Based on principle of non-maleficence, highly skilled nursing duties, task organization and team resource management can be successfully performed by simulated battlefield tactical combat casualty care training program that lead to major advances in both military and civilian trauma care.
