醫學知識與健康照護體系不斷進步,在疾病治療及照護也有創新突破,隨之衍生的問題則是民眾可能接受非必須、甚至對身體有害的醫療或照護程序。2012年美國內科醫師基金會(American Board of Internal Medicine Foundation)提出「明智選擇」(Choosing Wisely)活動,旨在促進臨床醫師與病人對話,避免病人接受非必須或有害醫療照護。自「明智選擇」運動在美國開始後,加拿大、荷蘭、英國等國家也陸續響應此活動。目前「明智選擇」運動風潮已逐步擴散至醫學之外的護理、藥學等領域。2017年美國的「明智選擇」運動已超過80個專業學會合作夥伴,制定多達520項明智選擇建議名單。其中,美國護理學會於2014年參與明智選擇活動,並發表首次以護理照護為導向的前五項低效益照護清單,至今共已發表20項明智選擇名單,其中5項與癌症相關。台灣「明智選擇」的相關研究始於2017年國家衛生研究院的「醫療資源使用之效益評估」論壇計畫,首度針對國內醫療資源使用之效益評估優先議題進行調查。然而,護理領域關於「明智選擇」方面的研究卻尚未起步。考量護理照護的多樣性,故擬先聚焦於蟬聯台灣十大死因榜首多年的癌症領域進行探討。此為國內癌症護理「明智選擇」的文獻回顧探討,期望本文能喚醒護理人員對癌症護理措施「明智選擇」的意識。

英文摘要

The traditional concept of more treatment equals better outcome, along with the advancements in medical knowledge and technology, resulted in some unnecessary or even harmful treatments. In 2012, the American Board of Internal Medicine (ABIM) Foundation proposed a "Choosing Wisely" concept that aimed to promote better communications between physicians and patients in order to avoid unnecessary or harmful treatments or cares. The Canada established the Choosing Wisely Canada in 2014, followed by Netherlands and the United Kingdom and into the field of nursing and pharmacy. In 2017, there were more than 80 professional associations that collaborated and created more than 520 items in the Choosing Wisely suggestion lists in the US. The America Nurse Association (ANA) first published a top-five list of low-val¬ue nursing-oriented practice in 2014. Currently, among the 20 items listed, five related to cancer care. The first study in Taiwan on Choosing Wisely, conducted by the National Health Research Institute in 2017, was "Evaluation of efficacy in healthcare resource utilization-Exploring of low-value healthcare." However, there was no relevant research specifically on nursing. Because cancer mortality continued the top 10 list in Taiwan, this study will focus on cancer-related nursing care. This study aimed to present the Choosing Wisely literatures in cancer nursing in order to raise the awareness of the Choosing Wisely concept among oncology nurses.
