本文為照護一位困難梭桿菌相關腹瀉的幼兒期病童護理經驗。筆者於2018年12月24日至12月30日護理期間運用Gordon十一項健康功能型態評估，經由觀察、會談、身體評估及實際照護等方式，確立個案有腹瀉、營養失衡：少於身體所需之護理問題及主要照顧者的知識缺失問題。故與照顧者建立治療性人際關係，採標準防護措施與接觸傳播防護措施，提供腹瀉照護及幼兒期兒童營養照護手冊，在醫療團隊共同合作下，規劃正確飲食照護計畫，改善照顧者對飲食資訊的知識不足，運用以家庭為中心的理念，兼顧個案及照顧者的生心理問題，在合宜的護理指導，提供正確的照護知識及技能，維護個案的健康。惟個案出院後，實難追蹤照顧者實際飲食的準備度；希冀此經驗分享，建議兒科護理人員臨床照護時應多方觀察照顧者的照護方式，及時改善錯誤認知及修正照護行為；篩選優質的衛教網站，讓3C時代的家屬能獲得正確的照護知識，共同維護兒科護理品質。

This article provides nursing experience of an early childhood patient with Clostridium difficile associated disease. During the nursing period from December 24 to December 30, 2018, the author used Gordon's eleven health function type assessments from observation, interviews, physical assessment and actual care to establish nursing problems, including diarrhea, nutritional imbalances: less than the body's care needs, and the lack of knowledge of the primary caregiver. The author established therapeutic interpersonal relationships with caregivers during the care process, adopted standard and contact transmission protective measures, provided diarrhea care measures and early childhood nutrition care manual. The medical team discussed and taught the caregiver on the proper dietary information, used the family-centered concept to address both patient and caregiver's physical and psychological concerns, and provided appropriate knowledge and skills in caring for this child. The author was unable to track the caregiver's compliance on dietary preparation after discharge from the hospital. The author suggested that the pediatric nurses should not only pay attention to the physiological problems but also assess and timely address the caregiver's knowledge and skills issues, and provide information on proper health education websites to family members so they can obtain appropriate care knowledge and jointly maintain the quality of pediatric care.
