交班作業是最頻繁的護理活動之一,完整交班可維持病人照護連續性,交班不完整除影響病人安全,亦增加醫療成本。本單位新進人員因交班壓力大與延遲下班等問題而離職,調查發現單位交班作業完整率僅66.9%、平均交班時間高達61.2分鐘,交班作業滿意度45.7%,故成立改善專案,目的為提升交班作業完整率與縮短平均交班時間。本專案透過制定ISBAR結構化交班標準、建置自動統整結構式資訊化交班作業、舉辦教育訓練,制作交班稽核機制等策略。專案成效良好,交班作業完整率提升到95.5%,平均交班時間縮短到29.3分鐘,交班作業滿意度88.7%。透過專案推行,可有效提升交班作業完整率,進而加強病人照護品質,並促進病人安全。

英文摘要

Shift change report is the most frequent work of nursing activities. Handover shift change reports completely can maintain the continuity of patient care. In contrast, handover incompletely will affect patient safety and increase medical costs. Due to heavy shift pressure and delays, new recruits in the unit resigned frequently. After the survey, we found that the completion rate of shift change report was only 66.9%, the average handover time was as high as 61.2 minutes, and the shift satisfaction was 45.7/100. Therefore, we started an improvement project of promoting the completion rate of shift change report and reducing the average handover time. We formulated ISBAR structured handover standards, established an automatic and integrated structured informatized handover system, hold education and training, and made a handover audit mechanism. The project has achieved great results. The completion rate of handover operations has increased to 95.5%, the average handover time has been shortened to 29.3 minutes, and the handover satisfaction scores have been 88.7/100. Through the implementation of the project, the complete rate of shift work can be effectively improved. Therefore, it enhanced the quality of patient care and promoted patient safety.
