背景:不恰當的資源配置、工作流程及人力不足,都會導致不良的護理工作環境。而護理工作環境的重新建構,使照護流程優化,對護理人員工作滿意度影響之研究仍闕如。目的:探討病房環境管理對護理人員工作滿意度的成效。方法:本研究為回朔性世代追蹤研究設計(retrospective cohort study),2020年7~9月在台灣北部某區域教學醫院隨機挑選某一急性病房為實驗組,運用5S手法推動環境管理,對照組(該院其他同性質的兩個急性病房)則維持原環境管理方式,並分別收集2020年3月(介入前)、2020年9月(介入後),及2021年9月(介入一年後)護理人員的工作滿意度。資料採卡方檢定和無母數曼-惠特尼U檢定進行組別差異比較,並運用Jonckheere's趨勢檢定檢視介入成效的趨勢變化。結果:兩組在介入前、後各時間點的護理人員工作滿意度及趨勢變化無統計差異,然而分析年資未滿3年的護理人員發現,實驗組介入病房環境管理1年後的專業自主發展工作滿意度較對照組好。結論:推動病房環境管理能讓服務年資未滿3年的護理人員的工作滿意度提升,期盼對工作留任之意向有正向的意義。 |
英文摘要 |
Background: Manpower insufficiency and inappropriate resource allocation, working environment or procedures would lead to an unfavorable working environment. However, little is known regarding the effectiveness of redesign and construction of the nursing work environment to optimize the nursing process on nurses' job satisfaction. Purpose: This study aimed to investigate the effectiveness of implementing ward environmental management on nurses' job satisfaction in general acute wards. Methods: This study randomly selected an acute general ward at a teaching regional hospital in northern Taiwan from July 2020 to September 2020 to implement the ward environmental management intervention (experimental group: adopted the steps of sorting, rectifying, cleaning, tidying and literacy, etc.; using multiple languages, photos/ figures, posters and labels to organize; items were managed by fixed-point, quantitative and regular audits); and the other two acute general wards maintained regular environmental management (control group). The Chi- Square Test and the Mann-Whitney U test were used to compare the differences between groups, and the Jonckheere's trend test was also used to examine the trend of the results. Results: There was no significant difference between groups in nurses' job satisfaction at baseline, post-intervention, and 1-year follow-up after the intervention. However, after one year, the nurses' job satisfaction in those with less than 3 years of nursing experience among experimental group was better than the control group. Conclusion: The implementation of ward environmental management can improve the nurses' job satisfaction in those with less than 3 years of nursing experience, anticipating that it has positive impacts on their job retention. |
