The factors affecting the post-radiotherapy quality of life of patients suffering head and neck cancer

台灣的頭頸癌死亡率很高，但鮮少研究探討這些病人接受放療後生活品質之相關因素。本研究目的主要是探討頭頸癌病人接受放射治療後的生活品質之相關因素。為觀察性研究，採方便取樣。評估工具為包括：生活品質評估量表為簡明版世界衛生組織生活品質問卷及歐洲生活品質量表，功能狀態為柯氏量表。共58位收案。研究結果發現治療後生活品質的生理健康範疇與治療前生理健康範疇呈顯著性相關。心理範疇顯著性相關因子為家庭收入狀況。社會關係範疇的顯著性相關因子為功能狀態，環境範疇顯著性相關因子為治療前環境範疇及家庭收入狀況。而工作狀態與歐洲生活品質量表達顯著性相關。建議改善病人治療前身體健康狀況及功能狀態，並運用社會資源以增加頭頸癌病人放射治療後的生活品質。

Although head and neck (H&N) cancer has a high mortality rate in Taiwan, less is known about the effects of radiotherapy treatment on patients' QOL and the factors that may contribute to these changes. The study aims to investigate the factors of quality of life (QOL) for H&N cancer patients after radiotherapy. An observational longitudinal study was used to analyse the QOL of patients. The World Health Organization Quality of Life Assessment (WHOQOL-BREF) and Euro QOL five-dimensional questionnaire (EQ-5D) were used to assess QOL. Performance status was measured by the Karnofsky Performance Scale (KPS) Index. 58 cases were included in the study. Physical health prior to radiotherapy had a significant correlation with physical health post-radiotherapy, whereas psychological health correlated significantly with family income. Social factors were significantly correlated to the KPS index. Employment status significantly correlated to ED-5D. It is recommended to improve the patient's physical health and functional status before treatment, and to use social resources to increase the quality of life of patients with head and neck cancer after radiotherapy.
