罹患糖尿病的風險隨年齡增加逐漸升高,而糖尿病的病因複雜,且牽涉甚廣。在身體組成改變方面,肌肉為處理人體餐後血糖的重要組織,故隨著老化肌肉量的下降會直接影響糖類代謝;亦會降低基礎代謝率。老年族群的身體活量降低合併基礎代謝率的減少,使能量過剩導致脂肪囤積。隨著體脂率的增高,循環中血脂異常與發炎細胞激素的增加,會直接損害肌肉、脂肪與肝臟等組織器官的胰島素敏感性;同時體內高血脂與發炎的環境,誘發循環系統中免疫細胞的活化。其中,免疫細胞促發炎表型的極化與促發炎細胞激素的分泌,受到細胞中發炎訊息傳遞路徑的影響。高脂與發炎細胞激素的合併作用之下,刺激發炎訊息傳遞路徑活化,進一步增加促發炎細胞激素的釋放。總體而言,身體組成改變、血脂異常、免疫細胞活化與促發炎激素累積等因素交互影響形成惡性循環,匯集眾多因素導致糖類代謝異常的發生。

英文摘要

Diabetes is a complex metabolic disorder disease. The prevalence of diabetes among older adults is high. In terms of changing in body composition, muscle is responsible for the majority of post-prandial glucose disposal. Thus, loss of muscle mass directly leads to decrease glucose uptake. On the other hand, overnutrition will cause accumulation of body fat that results in chronic inflammation. Dyslipidemia and increase of cytokines will directly impair insulin sensitivity of muscles, fats , and liver, also stimulate inflammatory signaling pathways in circulation immune cell. The polarization of the phenotype of immune cells and the secretion of cytokines are affected by the inflammatory signaling pathways. In conclusion, aging-associated disorders, such as changing of body composition, dyslipidemia, activation of immune cell, and chronic inflammation, may induce insulin resistance and increase risk for type 2 diabetes.
