Objectives. The aim of this study is to examine the factors related to incidence of colorectal adenomas. Methods. Cases were collected from the database based on the health examination survey in a regional teaching hospital in central Taiwan held during the period 2012 to 2013. A total of 5,263 eligible candidates were identified. Our results were analyzed using descriptive statistical analysis, chi-square tests, and logistic regression analysis. Results. A total number of 2,497 males and 2,766 females, with a mean age of 52.78±11.27 years old, were collected in our study. The prevalence of colorectal adenomas was higher in males than females, 11.2% and 7.1% respectively. The prevalence of metabolic syndrome (MS) was 31.6%. MS was more prevalent in females than males, 16.7% and 14.9% respectively. Over 80% of the subjects had at least one MS abnormality. MS was positively related to colorectal adenomas. Age, education level, vegetarian diet, smoking, alcohol consumption and MS abnormality were significantly related to colorectal adenomas. The three most typical indicators of MS were significantly related to the incidence of colorectal adenomas. Conclusion. The health authority should continue to promote the project for health checks, and encourage the eligible candidates aged 50 or above to maintain a high-fiber low meat diet and refrain from smoking, and alcohol consumption to reduce the risk of colorectal adenomas.

目的:在於探究影響大腸直腸腺瘤之相關因素,期能早期發現並降低大腸直腸腫瘤發生率進而進行切除。方法:對象為台中某區域教學醫院2012年及2013年健康檢查者,共收取5,263份有效樣本。採描述性分析、卡方分析及羅吉斯迴歸等進行統計分析。結果:男性2,497位,女性2,766位,平均年齡52.78±11.27歲。全體樣本大腸直腸腺瘤盛行率為男性盛行率多於女性,分別為11.2%、7.1%。代謝症候群盛行率為31.6%,女性盛行率多於男性,分別為16.7%及14.9%,八成以上的人至少有一項代謝症候群指標異常的情形。代謝症候群及大腸直腸腺瘤呈正相關。年齡、教育程度、素食、抽菸、喝酒、代謝症候群異常(血壓、三酸甘油脂及空腹血糖值)對大腸直腸腺瘤達統計上顯著相關。在代謝症候群指標中血壓、三酸甘油脂數、空腹血糖為大腸直腸腺瘤重要預測因子。結論:衛生當局持續衛教宣導,鼓勵50歲以上年齡民眾應定期接受大腸糞便篩檢,及多吃蔬果、減少紅肉、抽菸、喝酒等生活方式,期能降低大腸直腸腺瘤之風險。
