內政部空中勤務總隊一架AS365 N3型直昇機前往位於新北市石門地區海岸之德翔台北貨輪擱淺處,執行人員載運之任務,該機於接近貨輪準備進行人員吊掛時,墜毀於貨輪船身左側之海面上,直昇機全毀,造成兩人死亡,三人重傷。本文之失效分析流程包含背景資料蒐集、尾旋翼齒輪箱檢測與分析、軸承失效分析。在尾旋翼齒輪箱檢測方面,包括拆解尾旋翼齒輪箱、清洗齒輪箱零組件、巨觀觀察尾旋翼控制軸及軸承控制桿、拆解軸承控制桿之滾珠軸承、巨觀及微觀觀察軸承組件、軸承材料檢測等。最後綜合各項觀察與分析結果,據以研判軸承破壞模式,並找出軸承失效肇因。 |
英文摘要 |
On March 11, 2016, an AS365 N3 helicopter under National Airborne Service Corps (NASC), carried five members on board, including the pilot, copilot, crew chief, and two members of the Coast Guard. When preparing to hoist personnel near the cargo ship, the helicopter crashed into the sea and destroyed, two persons died and three persons were severely injured. Inspections include the following items: disassembling and cleaning the tail rotor gear box, inspecting macroscopic features of the control shaft and control rod, disassembling the control rod bearing, inspecting macro- and microscopic features of the bearing assembly, and further material tests. Finally, we find the root cause of the tail rotor gear box failure. |
