「胸悶」、「胸痛」、「氣短」是中醫門診常見的症狀，其原因非常複雜。一般而言，我們可以從現代醫學病理的角度來診斷，以及中醫學病因、病機的角度來辨證，進而決定治療的理、法、方、藥。本文特別要介紹在上述兩種方法之外，運用「沈-漢默氏中醫脈診系統」中一套獨立的脈象診斷思維，來探索與這些症狀息息相關的橫膈膜脈位出現陽性時的病因病機，並說明其相應的「開胸解鬱」的針灸治療對策。這是傳承自本派脈診系統的脈針相應之法，透過臨床反覆實踐，確實療效顯著。

＂Chest tightness＂, ＂Chest distress＂, and ＂Shortness of breathing(SOB)＂ are the common symptoms patient s present at the clinic of Chinese medicine. Those problems are derived from various origins. We may approach them from the aspect of biomedical science. We may also approach from the pathogenesis process of Chinese medicine, then make pattern diagnosis and decide the treatment through the analytic ways of theory, methodology, formula and interpretation. Apart from these, the author would like to introduce a unique model of treatment based on pulse diagnosis. According to Shen- Hammer Chinese Pulse Diagnosis Lineage System (SHCPDLS), there is a close relationship between the symptoms described above and the positive sign of Diaphragm Position. This will be explored in depth to illustrate the pathogenesis of this pulse quality and the associated treatment to ＂open the chest and resolve the stagnation＂ with acupuncture. This model of acupuncture based on pulse diagnosis is a knowledge passed down from ancestors of SHCPDLS. It has been proved to be effective by uncountable cases of clinical success over the years.
