排尿順利與否，目前在男人皆一下子想到攝護腺的問題，女性則子宮或膀胱下垂或生產所致膀胱尿道口的變形，實際上，牽連最多的反而是膀胱逼尿肌、膀胱內括約肌、膀胱外括約肌，而最難醫療的則為腰薦椎神經病變，即脊椎與薦椎脊髓神經病變所致的排尿障礙，這部份屬於脊椎脊髓神經內外科疾病，非泌尿科的疾病，目前西方神經內外科學界幾乎完全束手無策，反而中醫藥的療效極佳，此部份為「截癱」在泌尿神經的症狀，列在最後。

When a male has difficulty voiding, it is usually presumed that there is a problem with the prostate, and in a female the first thing that comes to most people's minds is uterine or urinary bladder prolapse or deformation of the external urethral orifice resulting from childbirth. But in actuality, the most common causes involve pathologies of the bladder detrusor, internal urethral sphincter, and external urethral sphincter. The most difficult to treat cause is lumbosacral neuropathies that lead to difficult urination. This latter cause can be classified as a condition for which neurologists or neurosurgeons would treat as opposed to the purview of a genitourinary specialist. However, Western medicine neurologists and neurosurgery specialists still do not have a means of providing effective treatment for these types of patients. On the other hand, Chinese medicine provides exceptional treatment for these patients. In Chinese medicine, this pathocondition can be classified as ＂paralysis (paresis)＂ of the nerves innervating the urinary tract.
