本文主要介紹「個人中心計畫」的內容、功能、與傳統轉銜計畫之異同、以及目前的實施情形。文中首先說明學生與家庭參與轉銜服務過程的重要性與實際參與上的困難,繼而引出如何以「個人中心計畫」來達到最佳的轉銜服務品質,並探討「個人中心計畫」於實務應用上的情形。

英文摘要

The purpose of this article was to introduce the concept, functions, characteristics, and implement of person-centered planning. The importance of personal as well as family involvement in the transition process was addressed, followed by the difficulties they might face during the process. Then the content of the person-centered planning toward the best transition service was described.
