背景:胎兒生長發育評估或是染色體中的三體症發生率最高為唐氏症(Down syndrome; trisomy 21)風險計算,在超音波下可以藉由胎兒肱骨長度(humerus length, HL)與股骨長度(femur length, FL)的測量來最為初步的判定是否有生長發育異常或是唐氏症高風險群的評估指標之一,因此,本研究的目的旨在建立一般族群14+0週至24+6週妊娠週數合併胎兒雙頂骨徑(Biparietal diameter, BPD)與胎兒肱骨與股骨長度之關係。方法:本研究採用2006年8月至2010年3月期間,於妊娠14+0週至24+6週在台灣台北中山醫院婦產科、台灣台北財團法人振興復健醫學中心婦產科與臺灣桃園和睦家婦產科接受常規產前胎兒評估檢查之個案,並將檢查時所測量之雙頂骨徑大小與肱骨與股骨長度加以統計分析。結果:2077位單胞胎妊娠個案符合本研究條件,本研究結果呈現出胎兒肱骨與股骨長度隨著妊娠週數與胎兒雙頂骨徑增加而增長,且統計結果發現二者之間具有線性關係。結論:產前超音波生長發育的評估或是唐氏症篩檢將胎兒肱骨與股骨長度納入評估項目,如此有助於增加生長發育異常與唐氏症篩檢敏感度。由於胎兒肱骨與股骨的長度會因地域及種族不同而有所差異,因此必須建立第二孕期胎兒肱骨與股骨長度適當的參考範圍。
