背景:手術後低體溫及寒顫是外科病人術後常見的問題,易導致術後合併症的增加,傷口癒合時間延長、感染率增加以及因顫抖產生之組織耗氧量的增加等問題。目的:探討接受全身麻醉腹腔手術者,術後低體溫狀況及其舒適度差異,藉由吹風式保暖機與暖照燈介入措施,對接受全身麻醉之腹腔手術者,影響術後低體溫之改善成效。方法:採類實驗研究設計,以北市某醫學中心恢復室127位術後病人為研究對象,術後每15分鐘追蹤體溫變化、自覺舒適度類比量表收集資料,以SPSS 22.0進行分析。結果:研究發現吹風式保暖機比暖照燈對於腹腔手術接受全身麻醉者,在術後第30分鐘後,保暖機達舒適度4分且體溫達36℃之累積百分比高於暖照燈。結論/實務應用:研究結果顯示合宜保暖護理措施,將會減少術後低體溫發生,預防術後合併症,可確保優質照護品質。 |

Background: Postoperative hypothermia and chills are common problems in surgical patients, and they may lead to an increase in postoperative complications, wound healing time, infection rate and tissue oxygen consumption generated by shaking. Purposes: To explore the relationship between postoperative body temperature and comfort after general anesthesia with abdominal surgery, and by the intervention measures of using air blower warm blanket or warm lamps, to improve postoperative hypothermia after general anesthesia with abdominal surgery. Methods: The quasi-experimental study was designed to investigate 127 postoperative patients in a recovery center of a medical center in Taipei City. The body temperature was measured every 15 minutes and the thermal comfort scale was used. All data were analyzed with SPSS 22.0. Results: The study shows that air blower warm blanket is better than warm lamps for the patients after general anesthesia with abdominal surgery. At 30 minutes after the start of the experiment, the cumulative percentage of patients in the air blower warm blanket group at the temperature above 36℃ and the thermal comfort level of 4 points was higher than the cumulative percentage of warm lamps. Conclusions/Implications for Practice: The results showed that appropriate warm care measures will reduce postoperative hypothermia, prevent postoperative complications at least, and ensure quality care. |
